Fill forms procedure<\/h5>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li>Obtain and fill out form called \u201c<em><strong>NOTICE OF MARRIAGE<\/strong><\/em>\u201d.<br \/>\n<strong>NOTE:<\/strong> Ensure names are boldly written, surnames underlined and passport photographs attached. A fee will be charged on submission of this form.<\/li>\n<li>Return to the registry after 21 days with already filled forms. Another set of forms would be giving \u201c<em><strong>AFFIDAVIT ON APPLICATION FOR CERTIFICATE<\/strong><\/em>\u201d.\u00a0 Attach a copy of both of your <em><strong>&#8220;Birth Certificate&#8221;<\/strong><\/em> when \u00a0submitting this form.
